We all dread that letter we get from the insurance company demanding our records or getting an EOB back from the insurance company that says they are not paying for the services that were billed because they were not medically necessary. I receive a lot of calls from doctors wondering why they are having records requested or why charges have been denied. Many times the reason is how the 1500 claim form was filled out. It is important to remember that the only way a doctor initially communicates a patient’s need for care to a third party payer is though the 1500 claim form. It is important to have a clean 1500 claim form. The 1500 claim form should tell a summarized story on the patient’s condition and how that patient is being treated.
