May chiropractic physicians sign forms for teacher sick leave, student sick leave, or student physical education modification? What is the full list of school forms that chiropractic physicians may sign?

The School Code permits chiropractic to sign some, but not all, medical forms required in Illinois schools.   Under the School Code, chiropractic physicians are authorized to sign forms for:

  • Student sick leave
  • Teacher sick leave
  • Modification or excusal from student physical education class

In addition, the Vehicle Code permits chiropractic physicians to sign off on school bus driver examinations.

Under the School Code, chiropractic physicians are not authorized to sign forms for:

  • Required student exams for pre-kindergarten, kindergarten, 6th grade and 9th grade
  • Certification of student medical condition requiring homebound tutoring
